COVID-19 Vaccination with Birth Certificate Registration Number
The concerned students of East West University who are 18 years old or above and do not have NID Number but they have Birth Certificate Registration Number are asked to fill in the prescribed format for COVID-19 Vaccination as requested by the University Grants Commission of Bangladesh (UGC).
We have already sent a link of Google Form to the students through email and SMS.
Due to time constraints and urgency of the issue, concerned students are requested to fill in the Format/ Google Form with the required information and submit the same by 17 September 2021. For any questions or queries, students may write to helpdesk-covid19vaccine@ewubd.edu.
